There is no charge for the following services:
- Supplying detailed report after each consultation (initial and review)
- Providing copies of your child’s BrainMap (one per child)
- Certificates of attendance at clinic for schools and employers
- Mailing of prescriptions
- Replying to your telephone queries about treatment-related problems
- Replying to fax queries about treatment-related problems
- Completing forms for schools, camps, and holiday programs so that medications can be administered
- Completing IPTAAS (Isolated Patients Travel Accommodation and Allowance Scheme) Forms
- Supplying letters to pharmacies for dispensing medicines at shorter intervals (if appropriate)
- Supplying letters to schools to support applications for special funding (if applicable)
- Supplying letters for Customs when medication is to be taken on overseas trips
There is a $30 fee for the following services. (Please make payment at the time of sending the form to the clinic):
- Completing Centrelink forms (other than IPTAAS)
- Completing Board of Studies Special Provisions application forms for School Certificate and High School Certificate examinations
- Completing University Admissions Council application forms for Education Access Scheme
- Completing forms not listed in previous section
There are special fees for providing reports for legal matters
- The fee for a medico-legal report depends on the time required to prepare the report and the complexity of the issue.
- Medico-legal reports will be supplied on receipt of a written request from a solicitor stipulating information required (preferably in point form).
